15 months  Measles, Mumps, Rubella (MMR)  
18 months   DPT, HIB, Prevnar  
2 years Check up  
3 years Check up  
4 years  PPD,DPT, Polio, MMR, Varicella CBC,Cholest Hear, Vision
After that, annually Check up, PPD, Flu   Hear, Vision
after age 10 Annual Check up EKGs & Spirometry (Lung Capacity)
New vaccines Adacel/ Boostrix, Gardasil, Hepatitis A, Menactra, Rotovirus
